N1 - Funding Information: The main funder of this Commission was the Bill & Melinda Gates Foundation, which covered travel, accommodation, and meals for the Commission meetings, and the development of background papers and research assistant time; additionally, SLD, LG, YRS, SSR, and AC were reimbursed for their writing time with grants.
