Respiratory arrest following tourniquet release after bilateral total knee replacement

Abstract

This is a case of 62 years old female with rheumatoid arthritis who was scheduled for one stage bilateral knee replacement. Regional anesthesia was planned and the patient received combined spinal - epidural anaesthesia. Surgery lasted for 3 hours. At the end of procedure, tourniquet on both sides was released with the interval of 10 minutes. Immediately, after the deflation of second tourniquet patient complained of nausea and vomiting and suddenly went into respiratory arrest, which was managed successfully. Different possibilities of respiratory arrest in this case are discussed in detail.
